The patient is often a middle-aged athlete playing tennis when he/she feels a sudden pain in the back of the upper calf followed by an inability to walk on the toes.  The pain is often described as that of having been shot or hit on the back of the knee.

Cause

This occurs when the knee is suddenly extended while the foot is dorsiflexed (toes and foot pointing up).  Another mechanism may be sudden dorsiflexion of the foot/ankle with the knee already extended.
